BIACUC Forms
These newest forms use tables instead of form fields. Some of the features of these forms include:
- These are now unprotected documents, which means that you have complete flexibility in formatting your text.
- The fields in which you enter your text still automatically expand to accomodate your text
- The check boxes do not "check" or "uncheck" with a mouse click like they used to. Now you must place your curser inside the box and enter an "x" in it.
These forms may continue to undergo modification, so please download a new form every time you need one to make sure you are using the most up-to-date version. These are Microsoft WORD document forms. Complete them using MS WORD and submit them to the IACUC Administrator as email attachements to: biacuc@indiana.edu.
